Are you an individual with a bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education? UPMC Western Behavioral Health at Safe Harbor is seeking a passionate Service Coordinator I to support our vibrant Blended Case Management team! Blended Case Management (BCM) is a supportive, individualized service that helps children, adolescents, and adults with mental health or behavioral challenges access community resources, coordinate care, and achieve personal recovery goals. As a Service Coordinator I, you will provide comprehensive case management services that focus on assessment and service planning, care coordination, advocacy, monitoring and evaluation, crisis and safety planning, education and support, and connecting individuals to community programs, resources, and services as needed. Working collaboratively with individuals and their support systems, you will help identify needs, remove barriers, and address social determinants of health through a recovery-oriented approach that integrates mental health and overall health care. Your efforts will center on developing targeted, measurable, and achievable goals that promote stability, independence, and success while empowering individuals to believe that recovery is possible and within reach for everyone. This position is eligible for the Erie County Human Services Cohorts (HSC) Initiative! This program is focused on growing and retaining the workforce for local agencies addressing mental health, substance use disorders, and social drivers of health. Designed to strengthen the county's behavioral health system, this program offers incredible benefits, including up to $45,000 in educational loan repayment, a $5,000 stipend and access to cohort sessions for skill-building and networking!
Key Information:
Schedule:
Ambulatory Monday- Friday 8:30am to 5:00pm with occasional flexibility to work non-traditional hours to meet the needs of clients.
On Call:
Availability Required for weekends, holidays, and evenings in a rotating system
Requirements:
Must have a completed bachelor's degree in above mentioned fields (pending graduates welcome to apply!) Valid driver's license

Responsibilities:
Coordinates mental health services, leading advocacy and promoting recovery through independent judgment and discretion. Develop and implement comprehensive service plans in collaboration with individuals, families and support systems. Facilitates interdisciplinary meetings, ensuring individuals needs are met and services are accessible. Provide support connecting individuals with internal and external resources. Maintain accurate records, participate in training and stay up to date on best practices and recovery-orientated services. Bachelor's Degree in a Human Services field such as Social Work, Psychology or Counseling required. Other fields of study may be considered with coursework of at least 12 credits in Human Services. One-year related experience preferred . Prior experience working in community settings preferred. Safe driving record required.
