North Dauphin Nursing and Rehab - Millersburg, PA North Dauphin Nursing and Rehab is seeking a dedicated and compassionate Social Worker to join our skilled nursing team in Millersburg, PA. In this role, you will serve as a vital advocate for residents, supporting their emotional well-being, assisting families, and facilitating smooth transitions throughout their stay. Key Responsibilities Complete comprehensive social service assessments and develop individualized care plans that address residents' psychosocial needs with realistic, measurable goals. Actively participate in pre-admission and admission processes to ensure appropriate placement and smooth transitions. Provide ongoing emotional support and crisis intervention to residents and families. Document behavioral, emotional, and psychosocial updates in compliance with facility policies and state and federal regulations. Facilitate individual and group support sessions as appropriate. Develop and coordinate volunteer programs tailored to resident interests and needs. Orient new residents and families to the facility, services, and available resources. Collaborate with the interdisciplinary team to ensure holistic, resident-centered care. Qualifications Bachelor's degree in Social Work or a related field preferred. Knowledge of long-term care social services practices, regulations, and compliance requirements. Strong interpersonal, communication, and organizational skills. Ability to work independently while contributing effectively to a multidisciplinary team. Demonstrated empathy, professionalism, and problem-solving ability.
