Case Manager II Family Preservation (Diversion Truancy Program) DCFCM • Family Preservation (FP) Norwood, PA Child/Youth Services Full Time, Varied
Hours/Pay Period:
75 Req # 10316 Diakon is one of the premier employers in Pennsylvania and Maryland. We are a non-profit organization which includes Diakon Child, Family, and Community Ministries.
General Responsibilities:
Responsible for serving as a resource person, case manager, counselor, and trainer to support the youth and families of the Diversion Truancy program. Maintains a caseload of five (5) youth and families. Establishes and maintains professional and supportive relationships with youth, birth families, referring personnel, treatment providers, school districts and others involved in the lives of children and youth. Schedules, completes paperwork necessary (ISP), and participates in initial Treatment Team meetings and quarterly reviews.
Education/Training:
A BA or BS degree in social service or a related field of study normally required
Experience:
A minimum of one (2) year's experience in a related child welfare or human service position normally required.
ADA:
ability to bend, stoop, sit, stand, reach and lift items weighing up to 50 lbs.; valid driver's license; visual and auditory skills.
