General Responsibilities:

Responsible for helping residents obtain supportive services from the community that are needed to enable independent living and aging in place. Refers and links the residents to supportive services provided by the Community such as, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness, and legal advocacy. Provides case management when service providers are not available through the general community. Creates a directory of providers for use by both development staff and residents.

Education/Training:

A Bachelor's degree in Social Work, Gerontology, Psychology, Counseling or related field of study normally required.

Experience:

A minimum of three (3) years' related experience normally required.

ADA:

Ability to ability to bend, stoop, sit, reach, and lift items weighing up to 25 lbs.; visual and auditory skills.
