UPMC is seeking a compassionate Hospice Social Worker to support patients and families in Lycoming and Clinton Counties. In this role, you will complete psychosocial assessments, coordinate individualized care plans, provide counseling, crisis intervention, and end-of-life support, and connect patients with community resources and benefits. You will collaborate with nurses, physicians, chaplains, and caregivers to ensure holistic, patient-centered hospice care. Responsibilities Conduct comprehensive psychosocial assessments for hospice patients and families Develop and implement individualized care plans in collaboration with the interdisciplinary team Provide counseling, grief support, and crisis intervention to patients and caregivers Educate families on hospice services, end-of-life options, and advance directives Coordinate community resources, financial assistance, and benefits for patients Participate in interdisciplinary team meetings and case conferences Advocate for patient rights, preferences, and culturally sensitive care Maintain timely, accurate documentation in the electronic health record Support bereavement services and follow-up for families Contribute to quality improvement initiatives within hospice services Required Skills Psychosocial assessment Hospice and palliative care practices Care planning and case management Crisis intervention Grief and bereavement counseling Community resource navigation Documentation and EHR systems Interdisciplinary team collaboration Patient and family education Cultural competency in end-of-life care
