UPMC is seeking a compassionate Social Worker to support patients and families across its integrated healthcare system. In this role, you will complete psychosocial assessments, provide counseling, and develop care plans that address medical, emotional, and social needs. You'll coordinate discharge planning, connect patients with community resources, and advocate for access to services. Working within an interdisciplinary team, you'll support complex cases, contribute to quality and population health initiatives, and uphold UPMC's mission of innovation, academic medicine, and community-focused care. Responsibilities Provide psychosocial assessments and counseling to patients and families across care settings Develop and implement care plans in collaboration with interdisciplinary healthcare teams Coordinate discharge planning, referrals, and linkage to community resources and support services Advocate for patients' rights, safety, and access to healthcare and social services Document assessments, interventions, and outcomes in the electronic medical record Participate in case conferences, quality improvement, and population health initiatives Provide crisis intervention, risk assessments, and safety planning as needed Educate patients, families, and staff on community resources and coping strategies Maintain compliance with legal, ethical, and regulatory standards of practice Engage in ongoing professional development and support UPMC's academic and research mission Required Skills Clinical psychosocial assessment Individual and family counseling Discharge planning and care coordination Knowledge of community and social service resources Crisis intervention and risk assessment Electronic medical record documentation Interdisciplinary teamwork Case management Knowledge of healthcare regulations and patient rights Cultural competence in patient care
