Job Description
Blue Peaks Developmental Services, Inc. is looking for a highly self- motivated, independent and dedicated individual to join our team as an Early Intervention Service Coordinator for our Early Intervention program. Duties include, but are not limited to, connecting families with services and supports for infants and toddlers with developmental delays and established conditions in the San Luis Valley. Responsibilities include becoming familiar with IDEA and Part C, scheduling screenings and evaluations, becoming familiar with EI data system, providing intake and service coordination for all families referred, developing plans and helping families transition to preschool settings. This position is 40 hours a week, Monday-Friday. Requirements include an experienced professional in case management with a BA in Sociology or Psychology and general knowledge of Early Childhood Development or an equivalent.
