Job Description
The Tuscarawas County Board of Developmental Disabilities (TuscBDD) arose from a grassroots effort by parents and concerned citizens in 1955 to provide educational services to 12 children with developmental disabilities through the creation of Starlight School. Over time, Starlight School has evolved into TuscBDD, which is a publicly funded county governmental entity offering an array of programs for over 700 eligible individuals. We officially became Tuscarawas County Board of Developmental Disabilities in October 2009. We are here to ensure that these residents have available to them the programs, services and supports necessary to help them enjoy a fuller, richer life. Regardless of our size, name, and funding sources, our product has remained the same ~ quality services to eligible individuals for all life stages: Early Intervention (Children) Preschool (Children) School Age (Children) Vocational Habilitation (Adult) Day Habilitation (Adult) Supported Employment (Adult) Activities (Adult) Transportation Service & Support Administration (SSA) Family Support Residential Under the direct supervision of the Family and Children First Council Manager, the Family and Children First Council (FCFC) Service Coordinator serves multi-system children and families within Tuscarawas County with the goal of developing a comprehensive plan of care. The incumbent will work closely with youth serving agencies across the county to develop necessary services and supports. Additionally, the FCFC Service Coordinator will work in conjunction with the FCFC Manager to lead Tuscarawas FCFC as it fulfills its mission of building community partnerships and identifying creative solutions for families. Essential Duties Initiates strength-based assessments of family needs, determines desired outcomes with the family, facilitates family involvement in decision making for their children. Facilitates coordination of services and collaborative planning among providers and the family. Provides support and advocacy options for the family. Maintains timely documentation related to Service Coordination activities. Monitors and records the progress toward outcomes identified in the service coordination plan. Facilitates and promotes an ongoing understanding of the Service Coordination Mechanism throughout the county. Demonstrates positive interpersonal relations in dealing with system providers and others so that productivity and positive relations are maximized. Maintains professional knowledge to assure competency as the Service Coordinator. Maintains productivity expectations on a monthly basis. Seeks additional funding streams to support Service Coordination activities. Identifies gaps in services and makes recommendations for potential solutions. Networks with community partners and works to facilitate strong relationships between services and families. Assists in maintaining an environment that enhances the trust and creativity of everyone involved in the program; i.e.; individuals, families, providers, co-workers, county officials and community. In collaboration with Noah's Hope Child Advocacy Center, completes information gathering from the family related to forensic interviews of alleged child victims, provides the family with support and advocacy options recommended by the Noah's Hope team, prepares forms and electronic video files as needed. Prepares or ensures preparation of required reports and documents, gathers and compiles information; submits documents to required authority in a timely manner. Demonstrates the ability to function effectively as a member of a team of co-workers at the worksite and other settings. Participates in meetings, assigned committees, in-services, public relations activities, and special projects and events as requested. Demonstrates regular and predictable attendance. Follows Board policies and procedures, applicable laws, rules, regulations and standards. Carries out related duties as requested.
