Your Role:

Making a Meaningful Difference As a Family Services Coordinator / Developmental Specialist (FSC/DS) with Aprendamos Early Intervention, you will serve as both a service coordinator and developmental provider for children participating in New Mexico's Family Infant Toddler (FIT) Program. You will partner closely with families and a multidisciplinary team to coordinate Early Intervention services, provide developmental instruction, monitor progress toward Individualized Family Service Plan (IFSP) outcomes, and connect families with resources that support the needs of the child and family. This role is ideal for someone who enjoys building relationships with families, coordinating services, working independently in the community, and making a meaningful difference during a critical stage of a child's development.

Key Responsibilities:

Provide Family Service Coordination and Developmental Specialist services in accordance with NM FIT Program requirements. Coordinate services identified within each child's Individualized Family Service Plan (IFSP) . Provide individualized developmental services to eligible children in their natural environments , including homes and community-based settings. Conduct regular face-to-face visits with families to discuss progress, address concerns, and provide family-centered strategies. Help families develop, implement, and review their child's IFSP and ensure identified services are coordinated and delivered appropriately. Coordinate and monitor services provided by therapists, subcontractors, medical providers, and other community partners. Initiate, attend, and facilitate Transdisciplinary Team Consultation Meetings (TTCMs) with members of the child's Early Intervention team. Initiate and coordinate required six-month reviews and annual IFSP meetings with families and team members. Conduct or participate in screenings, assessments, and comprehensive multidisciplinary evaluations as appropriate to training and qualifications. Facilitate initial and exit ratings for Early Childhood Outcomes . Monitor children's developmental progress and provide families with strategies to support developmental milestones within everyday routines. Assist families in identifying and accessing community resources, including housing, food, behavioral health, medical, and other family-support services. Coordinate with medical and healthcare providers as needed to support comprehensive care. Educate families about advocacy resources and empower them to build their own service coordination and advocacy skills. Facilitate transition planning for children and families as they prepare to exit Early Intervention services. Maintain accurate, timely, and compliant documentation in accordance with NM FIT Program and Aprendamos requirements. Complete and submit required Daily Notes, Delivered Service Logs, FSC Logs, billing documentation, and other program records within established deadlines. Maintain all required NM FIT certifications and training requirements necessary to provide and bill for FSC and DS II services. Meet established position benchmarks and program expectations. Maintain confidentiality and comply with HIPAA, organizational policies, and applicable Early Intervention requirements. Perform other duties as assigned.

Work Environment & Physical Requirements This is a community-based position requiring regular travel to provide services in children's natural environments, which may include family homes, childcare centers, and other community locations. The position requires the ability to: Travel throughout the assigned service area and safely operate a motor vehicle. Work in a variety of home, childcare, community, and office environments. Lift and carry materials or equipment weighing up to 40 pounds . Stand, walk, sit, bend, stoop, crouch, kneel, and reach as necessary when working with young children. Navigate stairs, ramps, uneven surfaces, and other conditions that may be encountered during community and home-based visits. Engage safely and appropriately with infants, toddlers, and families throughout service visits.
