We serve more than 68,000 clients per year throughout Middlesex, Somerset, Hunterdon, and Warren counties in NJ. Family Specialist 099 - Phillipsburg, NJ $42,500 per year Description Works as part of a multi-disciplinary team grounded in the housing first approach and coupled with an integrated service model to assist families in obtaining and retaining housing in Somerset, Hunterdon and Warren Counties. Responsible for establishing a trusting relationship with families and working with them to identify their service goals. Holistic planning approach to assist families with connecting to, navigating and coordinating needed health and social services. Rotating 24/7 coverage required with Case management and case conference with CP&P and other providers. Remote work is available after 3 months. Requirements Bachelor's Degree in related field required with 2 years' experience. Requires a flexible schedule including availability on evenings. Driver's license needed with an acceptable driving history.
