This is a fully remote opportunity for a dedicated professional passionate about serving diverse and elderly populations through coordinated, patient-cantered care. Key Responsibilities Care Management (70%) Collaborate with an interdisciplinary care team to address psychosocial needs such as financial concerns, transportation, and community support services. Coordinate member care needs and utilize community resources effectively. Provide consultation, education, and advocacy to members and families. Analyze assessments to identify individual needs and available resources. Monitor ongoing services and ensure appropriate utilization of support systems. Develop and maintain individualized service and care plans. Initiate referrals and follow-up for community-based support programs. Act as a liaison between members, families, and healthcare providers to achieve care goals. Maintain comprehensive documentation and case management statistics. Conduct or participate in case conferences and care coordination meetings. Minimum Qualifications Master's degree in Social Work (MSW) required.
