Interviewing patients and planning activities to meet the social and emotional needs of them and their families, and to facilitate the recovery psychologically, emotionally and physically. Coordinating with external agencies and home care nursing group for post-discharge care. Must be able to perform home visits if necessary. Job Qualifications License Clinical Licensure or Education Master's degree required and one of the following unrestricted TN licensures: LCSW or LMSW. Valid Driver's License required. Experience 3 years - Experience providing care coordination to persons receiving long-term care and/or home and community based services required Various immunizations and/or associated medical tests may be required for this position. Skills\Certifications Must be detailed oriented, deal well with ambiguity, demonstrate strong strategic thinking and creativity skills, be an effective communicator with both internal and external partners/customers Awareness of the impact of cultural/socioeconomic status pertaining to the use of healthcare and mental health care services.
