Position Description:

Home Care Partners has an opening for a licensed Social Worker in the D.C. Caregivers Institute program to provide case management for family caregivers; case management for home care clients who receive long term home health aide services and supervision of home health aides. Opportunity to help families keep their loved ones at home. This position offers a supportive team of professionals from diverse backgrounds. Clinical supervision provided. Position is FULL TIME, but a flexible hybrid schedule with opportunities for teleworking after initial orientation period.

Job Responsibilities:

One on one in-home meetings with D.C. families to evaluate their unique Age in Place needs Individual Counseling, information, referral and assistance for family caregivers, offering a generous array of program services and reimbursements. Collaborating with other HCP programs and staff to provide quality care. Conduct case management, evaluating and assessing both caregiver and care receiver's capacities to assist in the development of plans of service. Supervise and coordinate Home Health Aides to ensure quality services. Assists the client/family in utilizing community referrals for services including but not limited to Department on Disability Services, Veterans Administration, and other programs that provide services to persons with disabilities. Participation and facilitation of agency programs such as support groups and outreach events. Documentation and tracking of home visits. Conduct outreach to potential caregiver program participants at community sites and events Onboard caregiver participants to online learning platform and conduct monthly caregiver data reporting. Provide appropriate shadowing opportunities for social work interns to gain exposure to case management practices, caregiver support services, and community-based social work." Qualifications:

DC Social Work license and MSW (Master's of Social Work). Must have DC social work license by the date of employment. LGSW, LISW, or LICSW are acceptable.
