Job Description
Join a dedicated, mission-driven organization serving Utah communities for over 30 years as a PRN Social Worker in Home Health & Hospice in Moab & Grand County. Pay starts at $40/hr with DOE and licensing, plus travel stipends up to $125/day. This part-time role involves conducting psychosocial assessments, providing crisis and grief support, and assisting with care planning in a flexible schedule. Qualifications include a Master's in Social Work , active Utah licensure (LCSW preferred), and experience in hospice or home health. Must have reliable transportation and pass background checks.
