A Family, School, or General Case Manager coordinates the delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs. JOB DESCRIPTION AND POSITION REQUIREMENTS
This is a part-time position funded for approximately 10-12 hours per month. The position includes gathering and providing all required program paperwork and documentation to the Upward Bound Programs full-time staff member working with UB and UB Math & Science (UBMS) students who attend Reading High School. Upward Bound Programs is a federally funded program through the U.S. Department of Education. Penn State University currently has been awarded five UB/UBMS grants. Additional requirements include that the School Success Coordinator must be a current person working within the Reading School District.

This position requires the following clearances in addition to applicable background checks: PA State Police Criminal Background Check, PA Child Abuse History Clearance Form, and Federal (FBI) Fingerprint Criminal Background Check.

Pursuant to the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act and the Pennsylvania Act of 1988, Penn State publishes a combined Annual Security and Annual Fire Safety Report (ASR).
