Duties Conduct thorough assessments of our resident's needs. Develop personalized care plans and connect clients with appropriate resources such as Discharge planning or disability support. Provide crisis intervention and crisis management during emergencies or high-stress situations to ensure safety and stability. Support residents by facilitating communication, conflict management, and appropriate interventions tailored to individual circumstances. Maintain detailed documentation of resident interactions, care plans, and progress notes using the electronic medical record for accurate record-keeping. Collaborate with multidisciplinary teams—including healthcare providers, legal professionals, and community organizations—to coordinate comprehensive support. Deliver resident communication with empathy and clarity, ensuring culturally sensitive approaches when working with diverse populations. Qualifications Bachelor's or Master's degree in Social Work or a related field; licensure preferred but not required. Proven experience working with geriatric individuals or individuals with disabilities in high-risk populations. Excellent communication skills—both written and verbal—and the ability to engage effectively with residents from diverse backgrounds. Proficiency in navigating the electronic medical record as well as Microsoft Office applications for documentation and reporting purposes. Demonstrated crisis intervention skills along with experience managing conflict situations sensitively and professionally. Ability to work independently as well as collaboratively within a team environment dedicated to social work excellence.
