Supports Coordinator (Early Intervention Services) Position Summary Under limited supervision of the Early Interventionist Supervisor, the Supports Coordinator (Service Coordinator/Family Trainer) provides ongoing service coordination for children (birth to age 6) with developmental delays or disabilities and their families. This role ensures families can access needed services, understand their rights, and receive coordinated, high-quality early intervention supports that promote child development and family capacity. Coordinate across service systems Support transition planning DocumentationCompliance Maintain accurate records Complete service notes and documentation Ensure compliance with Medicaid and IDEA Part C Collaboration Work with families, providers, and agencies Participate in team planning and communication Qualifications Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology Early Childhood Education or Special Education Child Development, Family Studies, or Health-related field Communication Disorders or Rehabilitative Counseling Minimum 1 year of experience working with infants, toddlers, or children with disabilities (Alternative combinations of education and experience may apply) .

Preferred:

Experience with Early Intervention, IDEA Part C, or pediatric services Knowledge of
