development and reviews Participate in multidisciplinary team meetings Help families identify goals and outcomes Family SupportAdvocacy Educate families on rights and safeguards Promote caregiver competence and empowerment Support services in natural environments Service Monitoring Monitor services to ensure alignment withIFSP/FSP
Coordinate across service systems Support transition planning DocumentationCompliance Maintain accurate records Complete service notes and documentation Ensure compliance with Medicaid and IDEA Part C Collaboration Work with families, providers, and agencies Participate in team planning and communication Qualifications Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology Early Childhood Education or Special Education Child Development, Family Studies, or Health-related field Communication Disorders or Rehabilitative Counseling Minimum 1 year of experience working with infants, toddlers, or children with disabilities (Alternative combinations of education and experience may apply) .Preferred:
Experience with Early Intervention, IDEA Part C, or pediatric services Knowledge ofIFSP/FSP
development and child developmental milestones Experience working with families and community-based services Working Conditions- Community-based work
