Job Description

Funded by a US Dept of Education grant, the Lancaster Promise Neighborhood Success Coach duties and responsibilities will include, but are not limited to, playing a pivotal role in helping to ensure the well-being of students and families by providing comprehensive support services and supporting families. Working within a school setting, the coach will collaborate with various partners including students, families, teachers, and external agencies to address academic, social, emotional, and behavioral needs. By collaborating with school staff, community organizations, and other stakeholders, you will work to address barriers to student achievement and promote family engagement in the educational process. Success Coaches will be vital in increasing student academic success through case management.
Essential Duties and Responsibilities Assessment and Evaluation:
  • Regularly evaluate students' progress and adjust interventions accordingly.
  • Collaborate with teachers, counselors, and other professionals to gather relevant information to meet the student's and family's needs.
  • Conduct comprehensive assessments to identify the strengths, needs, and goals of families.
  • Develop a deep understanding of the unique circumstances and challenges facing each family, including socioeconomic factors, housing instability, healthcare access, and cultural considerations.
Case Management:
  • Develop and maintain individualized case plans for students based on assessment findings.
  • Coordinate and monitor the implementation of services and interventions outlined in the case plan.
  • Serve as the primary point of contact for students and their families, providing ongoing support and guidance.
  • Provide direct assistance and support to families in accessing community resources, such as food assistance programs, housing services, healthcare providers, and mental health resources.
  • Advocate on behalf of families to ensure they receive the necessary support and services to address their needs effectively.
Collaboration and Communication:
  • Foster effective communication and collaboration amongst school staff, parents, and external service providers.
  • Attending and actively participating in team meetings and parent/teacher conferences.
  • Provide regular updates and progress reports to relevant stakeholders regarding students' academic, social, and emotional development.
  • Promote parental involvement in school activities, parent-teacher conferences, and decision-making processes related to their children's education.
  • Collaborate with school staff to develop strategies for improving communication and building positive relationships between families and the school community.
Crisis Intervention and Support:
  • Respond to crises or emergencies involving students, providing immediate support and appropriate interventions.
  • Provide immediate support and intervention to families experiencing crises, such as domestic violence, substance abuse, or financial hardship.
  • Coordinate with school personnel and external agencies to develop safety plans and connect families with emergency assistance and support services.
Resource Referral and Coordination:
  • Establish and maintain partnerships with community organizations, social service agencies, and other relevant stakeholders to expand the range of support services available to families.
  • Coordinate referrals and facilitate access to external resources based on families' specific needs and preferences.
  • Participate in community outreach initiatives to raise awareness of available resources and promote collaboration among service providers. Qualifications

  • Excellent oral and written skills.
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint)
  • Willingness to travel and work evenings or weekends.
  • Knowledge and familiarity of the community, comfort in working in various neighborhood settings with diverse populations
  • Strong organizational skills and ability to manage multiple tasks.
  • Commitment to the goals, values and approach of Promise Neighborhood.
  • Willingness to work evenings.
  • Strong attention to details and deadlines is imperative, capacity to multitask.
  • Bachelor's degree preferred or equivalent experience
OTHER REQUISITES
From time to time the incumbent may be assigned special duties by a superior or, on a temporary basis, may be asked to assist other employees occupying a different position. Overtime work will be required on occasion. ADA REQUIREMENTS
Physical Requirements:
Tasks involve frequent walking, standing, and carrying (10-20 lbs.); manual dexterity in the use of fingers, limbs or body in the operation of shop and office equipment; may involve extended periods of sitting at keyboard.
