Social Worker - TLC/SNF

Job Description

Overview We are seeking a dedicated and compassionate Social Worker to join our team in a Skilled Nursing Facility (SNF) setting, providing vital support to residents and their families. In this role, you will play a key part in enhancing patient well-being through comprehensive psychosocial assessments, discharge planning, and community resource coordination. Your expertise will help ensure that residents receive personalized care aligned with clinical ethical guidelines and HIPAA regulations, fostering a respectful and supportive environment for individuals from diverse backgrounds. This paid position offers an exciting opportunity to make a meaningful difference in the lives of geriatric patients, those with developmental disabilities, and individuals requiring end-of-life care. Duties Conduct thorough psychosocial assessments and diagnostic evaluations to determine residents' emotional, social, and mental health needs. Develop and implement individualized care plans that incorporate behavioral therapy, clinical counseling, and motivational interviewing techniques. Coordinate community resources, hospice & palliative medicine services, and assist with discharge planning to ensure seamless transitions for residents. Provide individual and group counseling sessions focused on mental health, addiction counseling, dementia care, PTSD care, and Alzheimer's care. Collaborate with interdisciplinary teams—including nursing staff, physicians, therapists, and case managers—to deliver holistic patient care aligned with social work practice standards. Maintain accurate medical documentation and social work records using electronic medical record (EMR) systems while adhering to HIPAA confidentiality requirements. Engage in crisis intervention when necessary, managing acute situations with professionalism and sensitivity. Skills Proven experience in child & family counseling, behavioral health, mental health counseling, or medical social work within inpatient or outpatient settings. Strong knowledge of clinical research methods, data collection techniques, ICD coding (ICD-9/10), CPT coding, and Medicare regulations. Expertise in hospice care, end-of-life care, dementia care, Alzheimer's care, PTSD Care, and working with geriatric patients. Skilled in intake assessments, discharge planning, case management, crisis management, and working with individuals from diverse cultural backgrounds. Proficiency in EMR systems and medical terminology; familiarity with inpatient experience and nursing home environments preferred. Ability to conduct diagnostic evaluations using behavioral therapy approaches such as cognitive behavioral therapy (CBT) and applied behavior analysis (ABA). Excellent client communication skills; adept at individual/group counseling sessions; experienced in clinical interprofessional collaboration. Knowledge of HIPAA regulations and clinical ethical guidelines; committed to maintaining confidentiality and professional standards. Experience working with children or individuals with developmental disabilities is a plus; familiarity with assisted living settings is beneficial.
