Position Summary The Director of Social Services is responsible for planning, organizing, developing, and directing all functions of the Social Services Department. This role ensures that each resident's medically related emotional, social, and psychosocial needs are identified, addressed, and maintained in accordance with federal, state, and local regulations, as well as facility policies and procedures. Essential Responsibilities Lead and manage the Social Services Department to ensure high-quality resident-centered services. Assess residents' psychosocial needs and develop appropriate plans of care and interventions. Provide support, counseling, and advocacy for residents and their families. Coordinate admissions, discharges, transfers, and community resource referrals as needed. Participate as a member of the interdisciplinary care team to promote positive resident outcomes. Assist residents and families in navigating healthcare, financial, and social service resources. Ensure compliance with all federal, state, and local regulations governing long-term care facilities. Maintain accurate documentation and required records related to social services programs. Provide leadership, guidance, and support to social services staff. Qualifications Master's Degree in Social Work (MSW) from an accredited school of Social Work required. Active Social Work license in the state of practice required. Minimum of two (2) years of supervisory experience in a hospital, skilled nursing facility, long-term care setting, or other healthcare environment required. Geriatric and long-term care experience preferred. Membership in good standing with the Academy of Certified Social Workers (ACSW) and the National Association of Social Workers (NASW) preferred, where applicable. Strong leadership, communication, organizational, and interpersonal skills. Commitment to providing compassionate, resident-focused care.
