Oak Plains Academy is a comprehensive and highly structured 98-bed residential treatment program offering psychiatric residential treatment services to adolescents, struggling with emotional and behavioral issues. Residents receive an individualized treatment plan, geared to meet their personal needs. A professional team of psychiatrists, psychologists, social workers, therapists, nurses, and teachers serve hundreds of children and adolescents annually with the common goal of treating and healing trauma and building resiliency in the residents we serve. Located on 20 acres of rolling hills in a rural setting, about 45 minutes from Nashville, Tennessee, Oak Plains Academy campus features a full-size gymnasium, dining hall, separate academic building, and four dormitories. Job Summary:
The Case Manager is r esponsible for providing quality case management, admission support , utilization review activities and services to residents , their families and care providers. S erve as a member of treatment team supporting the organization's treatment program and philosophy and assure the deliverance of quality treatment to residents and their families. K nowledge of case management, discharge planning, age-specific growth and development. Crisis and behavior management, individual and family therapy theory practice, behavioral health and substance use.

Master's degree in Social Work required Licensed Clinical Social Worker, (LCSW) or e ligibility for licensure track in social work or related field acceptable.
