Licensed Social Worker - Winchester, TN -
Heritage Place Care & Rehabilitation Job Summary:
As a Licensed Social Worker at Heritage Place Care & Rehabilitation, you will play a vital role in providing high-quality care and support to our residents. We are seeking a dedicated and compassionate professional to join our interdisciplinary team and contribute to the overall well-being of our patients. In this full-time position, you will work closely with our healthcare team to assess, plan, and implement interventions to meet the social, emotional, and psychological needs of our residents.
Responsibilities:
Conduct social work assessments, develop and implement individualized plans, and provide support to residents and their families Identify and address social, emotional, and psychological issues that may impact a resident's ability to fully participate in their care plan Collaborate with the interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each resident Provide counseling, education, and support to residents and their families on topics such as coping with illness, grief, and loss Maintain accurate and thorough records of patient assessments, plans, and progress Participate in quality improvement initiatives and contribute to the development of policies and procedures that improve patient care
Requirements:
Bachelors degree in Social Work (BSW) or related field Current licensure as a Licensed Social Worker (LSW) in the state of Tennessee Minimum 1-2 years of experience in social work or a related field Ability to work in a fast-paced, dynamic environment and prioritize tasks accordingly Excellent communication and interpersonal skills Join our team today!
