Duties Conduct thorough patient assessments to evaluate emotional, social, and psychological needs, ensuring adherence to social work practice standards and clinical ethical guidelines. Provide individual, family, and group counseling sessions utilizing techniques such as motivational interviewing, cognitive behavioral therapy, and behavioral health strategies tailored for hospice and geriatric populations. Coordinate discharge planning and facilitate access to community resources, including assisted living facilities, mental health services, addiction counseling, and Alzheimer's or dementia care programs. Maintain accurate medical documentation within electronic medical record (EMR) systems while ensuring compliance with HIPAA regulations to protect patient confidentiality. Collaborate with interdisciplinary teams—including medical professionals, clinical researchers, and community organizations—to develop comprehensive care plans that address end-of-life needs. Engage in crisis intervention when necessary, managing complex cases involving PTSD care or individuals with developmental disabilities or autism spectrum disorder. Participate in ongoing training related to hospice & palliative medicine, ICD coding, Medicare policies, and acute care practices to stay current with industry standards. Experience Proven experience working with individuals from diverse cultural backgrounds in hospice care, mental health counseling, or clinical social work settings. Demonstrated expertise in medical social work practices such as intake procedures, diagnostic evaluation, case management, and discharge planning within inpatient or outpatient environments. Familiarity with home & community care services, inpatient experience, assisted living environments, and post-acute care settings. Strong background in working with geriatric patients and those with dementia or Alzheimer's disease; experience providing senior care is highly valued. Knowledge of medical terminology, ICD coding systems, EMR systems integration, and healthcare compliance standards including HIPAA is essential. Ability to perform crisis management effectively while maintaining professionalism under pressure; experience working with children or individuals with disabilities is a plus. Experience conducting clinical research or data collection related to behavioral health or hospice outcomes is advantageous for advancing patient-centered initiatives.
