Job Overview We are seeking a compassionate and dedicated Hospice Social Worker to join our interdisciplinary care team. In this vital role, you will provide emotional support, counseling, and resource coordination for patients facing end-of-life challenges, as well as their families. Duties Conduct thorough patient assessments to determine psychosocial needs, including mental health status and family dynamics. Provide individual, family, and group counseling sessions utilizing clinical counseling techniques such as motivational interviewing and cognitive behavioral therapy. Coordinate community resources to support patients' social, emotional, and practical needs, including housing, financial assistance, and spiritual support. Facilitate discharge planning and transition to hospice or post-acute care services by collaborating with medical teams and caregivers. Maintain accurate medical documentation in electronic medical records (EMR) systems, ensuring compliance with HIPAA regulations and social work practice standards. Engage in crisis intervention when urgent psychosocial issues arise, including managing grief, trauma, or behavioral health crises such as PTSD or dementia-related behaviors. Participate in interdisciplinary team meetings to align care plans with clinical ethical guidelines and hospice & palliative medicine best practices. Requirements Bachelor's or Master's degree in social work (BSW or MSW) from an accredited program; licensure or certification as required by state law. Proven experience working with geriatric patients, individuals with disabilities, dementia care, Alzheimer's care, or end-of-life care settings. Strong knowledge of hospice care principles, medical terminology, ICD coding, Medicare regulations, and clinical ethical guidelines. Demonstrated skills in behavioral health interventions such as behavioral therapy, crisis management, and mental health counseling. Familiarity with EMR systems for documentation and data collection; experience with inpatient or outpatient clinical settings preferred. Ability to work effectively with diverse cultural backgrounds and populations requiring specialized support like autism spectrum disorder or developmental disabilities. Valid CPR/BLS certification; experience in hospital or acute care environments is highly desirable. Excellent client communication skills coupled with the ability to collaborate within an interprofessional healthcare team for optimal patient outcomes.
