Are you passionate about making a difference in the lives of children and families? Passion for Families is seeking a reliable Case Manager who's ready to step in, take initiative, and help shape the future of our growing agency.
Position Overview:
The Case Manager oversees the care and services provided to children in foster care, pregnant or parenting youth, youth requiring transition services, and children with Intellectual and Developmental Disabilities (IDD) or Autism Spectrum Disorder (AU). This role ensures the development and implementation of individualized care plans in compliance with T3C guidelines.
Key Responsibilities- Care Coordination:
Develop and manage individualized service plans for children, ensuring alignment with
CANS 3.0
Assessments and regulatory requirements. Conduct regular home visits and maintain ongoing communication with caregivers, mentors, and service providers. Ensure timely and appropriate access to medical, behavioral, and educational services through collaboration with STAR Health and other providers.
Monitoring and Documentation:
Monitor child progress and update care plans based on outcomes and changing needs. Maintain accurate documentation of services and progress in compliance with T3C guidelines.
Quality Assurance:
Participate in Continuous Quality Improvement (CQI) initiatives to enhance service delivery. Support compliance with staffing ratios and regulatory standards.
Qualifications:
Education:
Bachelor's degree in social work, psychology, or a related field.
Experience:
Experience in child welfare, foster care, or social services preferred.
Skills:
Strong case management, organizational, and communication abilities.
Required Trainings:
Passion for Families Policies and Procedures. Passion for Families Orientation. T3C System Overview and Compliance. Trauma-Informed Care Practices. Universal Trafficking Prevention Training.
CANS 3.0
Assessment Training. Documentation and Reporting.
Work Environment:
Field-based with regular home visits and occasional office work. Flexible schedule required, including evenings and weekends as needed. Must have a valid driver's license and reliable transportation
Job Type:
Full-time Pay:
$40,000.00 - $42,000.00 per year
