This is an excellent opportunity for an MSW interested in developing expertise in renal care. What You'll Do

• Complete psychosocial assessments and provide ongoing patient and family support.
• Address barriers involving transportation, insurance, finances, employment, treatment adherence, and psychosocial needs.
• Provide counseling, education, advocacy, resources, and appropriate referrals.
• Support patients adjusting to ESRD, dialysis, home therapies, transplantation, and changes in treatment modality.
• Collaborate with nursing, the Renal Dietitian, nephrologists, and other interdisciplinary team members.
• Participate in interdisciplinary assessments and plans of care.
• Document interventions and follow-up in the electronic medical record.
• Support quality initiatives focused on patient experience, adherence, retention, and outcomes.

• Master's degree in Social Work (MSW) - Required
• Degree must meet applicable ESRD Social Worker requirements.
• Appropriate Texas social work licensure/credentialing as applicable.
• Must meet applicable CMS and Texas ESRD qualification requirements.
• Dialysis experience is not required.
• Fluent Spanish-speaking skills preferred, but not required.
• Excellent customer service, communication, problem-solving, organization, and follow-through.
• Comfortable learning dialysis workflows and electronic medical records.
