Duties Conduct comprehensive assessments to identify clients' needs related to behavioral health, child welfare, addiction, and other social issues Develop personalized care plans that incorporate crisis intervention, motivational interviewing, and behavioral health strategies Refers to services such as addiction counseling, crisis management, mental health support, etc. Coordinate with community agencies to facilitate access to resources like childcare, housing assistance, and healthcare services Document all client interactions thoroughly while maintaining confidentiality and compliance with legal standards Minimum Requirements Bachelor's degree from an accredited college or university with a major in social work, psychology, or a field related to human behavior Valid Texas Driver's License Join us to be part of a passionate team committed to fostering positive change!
