Role:
As a Social Worker at Levelland Nursing, you will be responsible for: Conducting social assessments and developing individualized plans to meet the unique needs of our patients and residents Coordinating with healthcare teams to ensure that patients and residents receive comprehensive care Providing emotional support and counseling to patients and their families Facilitating communication and collaboration between patients, families, and healthcare providers Identifying and addressing social determinants of health that impact patient outcomes
Responsibilities:
Provide social services to patients and residents, including assessments, counseling, and support Develop and implement social plans, including referrals to community resources and services Coordinate with healthcare teams to ensure that patients and residents receive comprehensive care Identify and address social determinants of health that impact patient outcomes Maintain accurate and timely records of patient interactions and services provided
