Job Summary Longview Regional Medical Center is seeking an experienced Licensed Social Worker - Care Manager (LSW) to join our team on a PRN basis. This position will initially provide temporary maternity leave coverage—working several days per week during September, October, November, and December—and will transition into an ongoing PRN role working 1-2 days per week afterward. In this role, you will coordinate discharge planning, manage transitions of care, and oversee case management activities to ensure optimal patient outcomes and seamless continuity of care. We are looking for a dedicated Social Worker—preferably with recent acute care experience to allow for an accelerated orientation—to collaborate with our interdisciplinary care teams, conduct utilization reviews, and support high-quality patient care in Longview, TX.
Key Responsibilities Care Coordination & Discharge Planning :
Develop and execute comprehensive discharge plans, arrange post-hospital placements, and coordinate social services tailored to individual patient needs.
Utilization Review :
Conduct daily medical record reviews to evaluate the appropriateness of admissions, continued hospital stays, and diagnostic service utilization.
Interdisciplinary Collaboration :
Facilitate and participate in Interdisciplinary Team (IDT) meetings to streamline care transitions, identify avoidable hospital days, and resolve care delivery challenges.
Physician & Escalation Liaison :
Refer non-matching cases to physicians, managers, or RN Care Managers for timely intervention and clinical oversight.
Community Connection :
Serve as the primary liaison with community agencies to maintain strong partnerships and support smooth post-discharge care.
Protective Services :
Identify and appropriately report cases to Child/Adult Protective Services in compliance with legal, state, and ethical mandates.
Patient & Family Advocacy :
Provide guidance, support, and professional assistance to patients and their families regarding post-acute care options.
Documentation & Compliance :
Maintain precise, timely case management documentation in compliance with hospital policies, state/federal regulations, and Joint Commission standards.
General Duties :
Perform other duties as assigned while maintaining regular and reliable attendance.
Qualifications & Requirements Education & Licensure:
Education :
Bachelor's Degree in Social Work (BSW) required; Master's Degree in Social Work (MSW) preferred.
Licensure :
Active state license as a Licensed Clinical Social Worker (LCSW) OR Licensed Master Social Worker (LMSW) required.
Experience :
2-4 years of social work or case management experience in an acute care hospital setting required. Recent acute care hospital experience preferred to ensure a quick transition into coverage.
Availability Requirement:
Ability to work several days per week from September through December to cover a maternity leave, followed by 1-2 days per week ongoing.
Knowledge, Skills, & Abilities:
Deep understanding of acute care case management principles, utilization review, and discharge planning protocols. Thorough knowledge of federal, state, and Joint Commission compliance standards. Proficiency with Electronic Medical Record (EMR) systems and clinical documentation. Strong assessment, problem-solving, and organizational skills in a fast-paced clinical environment. Outstanding communication skills to collaborate effectively with patients, families, physicians, and multidisciplinary care teams.
