Job Description

We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families in navigating various behavioral health challenges. This position requires strong interpersonal skills and a commitment to helping clients achieve their goals through effective case management practices. Case Manager is responsible for coordinating and monitoring trauma-informed, individualized services for children placed in a General Residential Operation (GRO) setting. The case manager ensures that all care is aligned with the T3C framework, promoting safety, permanency, and well-being through interagency coordination and family engagement. Key Responsibilities
  • Serve as the lead in developing, monitoring, and updating Individualized Service Plans (ISPs) with input from youth, families, and interdisciplinary teams.
  • Coordinate all required services including:
  • Physical and mental health care (including STAR Health)
  • Education and vocational services
  • Court-related requirements (e.g., court reports, permanency hearings)
  • Family finding and engagement efforts
  • Ensure routine and emergency service delivery is timely and aligns with the
CANS 3.0
assessment and other clinical indicators.
  • Collaborate with DFPS or SSCC caseworkers, licensed professionals, family members or fictive kin, behavioral health providers, and school districts.
  • Lead or participate in Multidisciplinary Team (MDT) meetings, case staffings, and transition planning conferences.
  • Maintain up-to-date and accurate documentation in accordance with Minimum Standards, GRO policy, and the T3C Blueprint.
  • Monitor progress toward discharge planning goals, permanency, and family reunification when appropriate.
  • Coordinate aftercare planning and post-discharge follow-up to ensure continuity of support.
Minimum Qualifications Education:
  • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a related field
Substitution:
In lieu of a degree, a minimum of 5 years of verified direct child welfare experience in residential or foster care may be accepted.
Experience:
At least 2 years of experience working in a residential child care or child welfare setting preferred.
  • Familiarity with
DFPS, SSCC, STAR
Health, and trauma-informed care is strongly preferred.
Licensure/Certification:
  • Must meet T3C Tier I personnel qualifications.
  • Valid Texas Driver's License and clean driving record.
  • Must complete or have completed trauma-informed care training as required by DFPS.
Knowledge, Skills, and Abilities Ability to work collaboratively with internal teams and external systems. Ability to advocate effectively for children's rights and individualized services. Excellent organizational, time management, and documentation skills. Ability to function effectively in high-stress, crisis-driven environments while maintaining child-focused decision-making.
