Location:
Beaumont, TX. Looking for meaningful work where you can truly support patients and families?. Concho Hearts Hospice is seeking a compassionate PRN Medical Social Worker (LMSW or LBSW) to support patients and families in the Beaumont and surrounding area. Hospice social work is about more than assessments; it's about helping families navigate one of life's most difficult journeys with support, guidance, and compassion. Our Social Workers play a vital role in providing emotional support, connecting families with resources, and helping patients maintain dignity and comfort. What You'll Do:
Assess the psychosocial needs of hospice patients and their families. Ability to provide compassionate emotional support to patients and families during end-of-life care. Assist families in identifying community resources and financial or social support services. Collaborate with the RN Case Manager and interdisciplinary team to develop individualized care plans. Participate in interdisciplinary team meetings (IDT) to support patient care. Assist with bereavement planning and coordinate with the Bereavement Coordinator as needed. Document assessments and interventions in the EMR system.
What We're Looking For:
Licensed Social Worker in Texas (LMSW or LBSW). Master's or Bachelor's degree in Social Work from an accredited program. Minimum 1 year of social work experience in healthcare, hospice, or long-term care. Hospice experience preferred but not required. Strong communication and counseling skills. Ability to work independently while collaborating with a care team. Reliable transportation and willingness to travel locally.
