Clinical Director / Administrator Position Summary Stonewood Hospice is seeking a compassionate, professional, and dedicated Social Worker to join our interdisciplinary hospice care team. The Social Worker provides psychosocial support and counseling to hospice patients and their families, helping them navigate the emotional, social, financial, and practical challenges associated with serious illness, end-of-life care, and bereavement. The ideal candidate demonstrates empathy, sound clinical judgment, strong communication skills, and a commitment to providing patient- and family-centered care while honoring each individual's goals, values, culture, and preferences. Essential Responsibilities Conduct comprehensive psychosocial assessments of patients and families and identify social, emotional, environmental, and financial needs. Develop and implement individualized psychosocial care plans in collaboration with the patient, family, interdisciplinary team, and other appropriate professionals. Provide supportive counseling, emotional support, crisis intervention, and education to patients and family members. Assist patients and families with adjustment to illness, changes in functional status, caregiver stress, grief, loss, and end-of-life concerns. Participate actively in interdisciplinary team meetings and communicate relevant psychosocial findings and recommendations. Advocate for patient and family needs, preferences, rights, and access to appropriate community resources. Connect patients and families with community agencies and resources related to financial assistance, transportation, housing, food, insurance, advance planning, caregiving, and other identified needs. Assist patients and families with advance care planning and facilitate appropriate referrals when needed. Provide education regarding hospice services, goals of care, available resources, and end-of-life support. Support families with anticipatory grief and coordinate with bereavement services as appropriate. Maintain accurate, timely, and compliant clinical documentation in accordance with organizational policies and applicable regulations. Provide home, facility, or other appropriate visits based on patient and family needs. Respond appropriately to psychosocial emergencies and collaborate with the clinical team to develop effective interventions. Maintain professional relationships with patients, families, caregivers, healthcare professionals, community organizations, and referral partners. Protect patient confidentiality and comply with all applicable privacy, ethical, and regulatory requirements. Participate in quality improvement initiatives, staff meetings, education, and other organizational activities as assigned. Perform other duties consistent with the role and the needs of Stonewood Hospice. Qualifications Appropriate social work licensure or certification required by the applicable state and position. Previous experience in hospice, palliative care, healthcare, geriatrics, behavioral health, or a related setting preferred. Knowledge of psychosocial issues associated with serious illness, death, dying, grief, and bereavement. Strong assessment, counseling, communication, advocacy, and crisis-intervention skills. Ability to work effectively as part of an interdisciplinary team. Demonstrated ability to maintain professional boundaries and exercise sound clinical judgment. Strong organizational skills and attention to detail. Ability to work independently, prioritize responsibilities, and manage a flexible schedule. Reliable transportation and the ability to travel to patient homes and/or care facilities as required. Current driver's license and appropriate automobile insurance, if required for the position.
