- MSW to join our hospice interdisciplinary team.
This position is ideal for a social worker who values flexibility and enjoys supporting patients and families through the emotional, social, and practical challenges associated with end-of-life care. Duties Complete initial and ongoing psychosocial assessments and PRN social work visits based on patient and family needs and the hospice plan of care. Provide emotional support, counseling, crisis intervention, and end-of-life support to hospice patients and their families. Assess caregiver coping, family dynamics, available support systems, financial concerns, and psychosocial barriers that may affect the patient's care. Assist patients and families with identifying and accessing appropriate community resources and support services. Provide education and assistance to patients and families regarding advance care planning and assist families who need help understanding and completing DNR documentation , while coordinating with the hospice clinical team and physician as appropriate. Provide support and education regarding end-of-life planning, funeral arrangements, placement needs, caregiver resources, and other psychosocial concerns as applicable. Collaborate with nursing, hospice physicians, chaplain, bereavement services, and other members of the interdisciplinary team. Participate in mandatory scheduled IDG meetings and contribute relevant psychosocial updates to the patient's interdisciplinary plan of care. Promptly communicate significant changes, safety concerns, caregiver concerns, or newly identified psychosocial needs to the hospice clinical team. Complete accurate and thorough documentation for all patient and family encounters. Submit completed visit documentation within 24 hours of the patient visit. Maintain patient confidentiality and comply with hospice policies, HIPAA requirements, and applicable federal and state regulations. Maintain dependable communication and availability for accepted PRN assignments. The MSW will collaborate with nursing, the hospice physician, chaplain, bereavement services, and other members of the interdisciplinary team; participate in IDG meetings as required; communicate significant changes or concerns to the hospice team; and complete timely and accurate documentation following each encounter. Qualifications Master's degree in Social Work (MSW) from an accredited institution; licensure or certification as a Licensed Clinical Social Worker (LCSW) preferred. Proven experience working with hospice care patients or in palliative medicine settings; experience with geriatrics and dementia care highly desirable. Strong knowledge of hospice & palliative medicine principles, medical terminology, ICD coding, and clinical ethical guidelines. Familiarity with Axxess EMR systems and medical records management; proficiency in data collection and clinical research is a plus. Excellent patient assessment skills combined with the ability to provide compassionate mental health counseling across diverse cultural backgrounds. Ability to perform crisis management effectively while adhering to social work practice standards and HIPAA compliance. Experience working with individuals with disabilities or developmental disorders; knowledge of Alzheimer's and PTSD care is beneficial.
