Responsibilities Conduct thorough patient assessments to identify social, emotional, and environmental needs, utilizing clinical research methods and data collection techniques. Provide child & family counseling, behavioral therapy, and mental health counseling tailored to individual circumstances. Coordinate community resources such as housing assistance, transportation services, and support groups to enhance patient care plans. Engage in discharge planning and end-of-life care discussions with patients and their families, ensuring compliance with hospice & palliative medicine standards. Document medical records accurately using EMR (Electronic Medical Records) systems while adhering to HIPAA regulations and medical documentation standards. Collaborate with interdisciplinary teams including healthcare providers to develop and implement personalized treatment strategies. Manage crisis intervention situations effectively, providing immediate support for patients experiencing acute mental health or behavioral health crises. Qualifications Valid LMSW licensure with active certification in social work practice standards; additional licenses or certifications in hospice care or addiction counseling are a plus. Proven experience working with individuals from diverse cultural backgrounds across inpatient, outpatient, or community settings. Strong knowledge of medical terminology, ICD coding, Medicare policies, and clinical ethical guidelines related to social work practice. Demonstrated expertise in behavioral health interventions such as cognitive behavioral therapy (CBT), motivational interviewing, and applied behavior analysis (ABA). Experience working with geriatric patients, individuals with developmental disabilities or dementia care needs; familiarity with Alzheimer's and PTSD care preferred. Ability to perform patient assessment and diagnostic evaluation within acute care or home & community care environments. Excellent client communication skills coupled with the ability to collaborate effectively within clinical interprofessional teams.
