Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them. Community Healthcare of Texas is currently recruiting for a Social Worker.
POSITION SUMMARY
To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients' needs/wishes with team members, family members, caregivers, and other providers.
ESSENTIAL FUNCTIONS
Assess the psychosocial needs of the patient and family members Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues Contribute to and execute the Plan of Care with the family/patient Record daily documentation of visits and contact in accordance with Policy Provides referrals to community resources Performs in-office/non-clinical administrative duties Performs other duties as assigned
POSITION QUALIFICATIONS
Master's degree in Social Work required Texas State license required Reliable transportation and a valid driver's license. Hospice or healthcare experience preferred.
