Job description Rocky Mountain Care - Cottage on Vine is seeking an energetic and dynamic individual as a full-time Social Services/Resident Advocate in our skilled nursing facility. We are looking for someone who is experienced in healthcare and mental health to join our amazing team and provide social services and DC planning for our residents under our care.

Responsibilities include:

Provide social services to meet the needs of the residents and family Demonstrate ability to engage interdisciplinary team, staff, resident and family members, and understands and can provide the emotional resource necessary for end-of-life care as needed Attend all IDT and mandatory meetings, and communicates effectively with interdisciplinary team members Complete all required documentation in compliance with rules and regulations, including documentation of communication with the resident/families, advising for long-range planning and decision-making, community resource planning, short-term therapy, etc. Mental Health assessments and referrals Provide timely and effective discharge planning Other duties as assigned

Position Requirements:

Excellent time management and organizational skills Excellent punctuality and attendance Nursing home experience is preferable Experience in Discharge Planning is preferable Ability to multitask and meet deadlines Must possess solid knowledge of a clinical environment Patience and professionalism A positive attitude and team approach
