Join a dedicated, mission-driven nonprofit providing compassionate home health and hospice care in Moab & Grand County. We seek a PRN Social Worker to support patients and families through assessments, crisis support, grief counseling, and care planning. This part-time role offers starting pay at $40/hr+ DOE, a travel stipend up to $125/day, and weekly team meetings on Tuesdays at 11 AM. Conduct psychosocial assessments in home and hospice settings. Collaborate on care plans with interdisciplinary teams. Assist with legal, financial, and end-of-life planning. Provide emotional support and advocacy. Maintain compliant documentation. Qualifications include a Master's in Social Work, Utah licensure (LCSW preferred), and experience in hospice or home health. Must have reliable transportation, pass background and drug screens.
