Pure Faith House (PFH) operates two therapeutic group homes serving youth with mental and behavioral health needs in Hampton, Virginia — PFH I for girls (ages 12-18) and PFH II for boys (ages 10-17). Our mission is to provide a safe, structured, faith-informed environment where every young person can heal, grow, and prepare for a stronger future. Position Summary The Case Manager coordinates the full continuum of care for each resident — from intake and assessment through service planning, treatment coordination, and discharge. This role is the connective link between youth, families, referring agencies, clinical staff, and community providers, making sure every young person's plan of care is followed, updated, and moving them toward their goals. The Case Manager reports to the Program Director and partners closely with the Clinical Director, Child Care Supervisor, and Youth Support Coordinator. Key Responsibilities Complete intake, admissions paperwork, and initial assessments for new residents. Develop, implement, and update Individualized Service Plans (ISPs) in collaboration with the youth, family, clinical team, and referring agency. Conduct and document quarterly service plan reviews, progress notes, and required updates. Coordinate medical, dental, educational, therapeutic, and community-based services for each resident. Serve as the primary point of contact for referring agencies, case workers, courts, schools, and family members. Facilitate and document family engagement, visitation, and family team meetings. Prepare discharge plans and aftercare supports to ensure smooth transitions home or to the next level of care. Maintain complete, accurate, and audit-ready resident records and case files. Partner with the Clinical Director on treatment planning and crisis response. Support youth in developing life skills, independent living goals, and community connections. Attend and contribute to staff meetings, treatment team meetings, and trainings. Qualifications Applicants must meet ONE of the following: 1. A master's degree in social work, psychology, or counseling; OR 2. A bachelor's degree in social work or psychology with documented field work experience (under supervision by qualified program staff); OR 3. A bachelor's degree in any field and at least three years of professional experience working with children or adolescents. Additional Requirements Must be at least 21 years of age. Valid driver's license, reliable transportation, and clean driving record. CPR, First Aid, and Medication Administration certification (or ability to obtain within 30 days). Strong writing, documentation, organization, and communication skills. Comfort using computers, email, and electronic recordkeeping systems. Preferred Experience Prior case management experience in a children's residential, therapeutic, foster care, or behavioral health setting. Familiarity with trauma-informed care, ISP development, and Medicaid/MCO documentation. Experience working with referring agencies, courts, and school systems. Bilingual (English/Spanish) a plus.
