Social Work Case Manager -
Laurel Fork, VA Pay:
$19.00 - $25.00 per hour Job description: Tri-Area Community Health Center in Laurel Fork is seeking a full-time Social Work Case Manager/Care Coordinator. Experience and a minimum of an associate's degree in Human Services or a related field required. The position is responsible for coordinating patient access to care needs. Provides additional support to individual patients. Referrals for specific programs as well as general consults, indigent care, and maintaining a broad referral network through outreach are all components of the position. Collaborative communication and positive working relationships with clinic staff, behavioral health and medical providers, and community partners are necessary skills.
