Job Description
Overview The Family Planning Coordinator (FP Coordinator) will carry out responsibilities which insure high quality, well-coordinated patient care for the clinic according to Title X Family Planning guidelines and the mission of CBHA. The FP Coordinator will work closely with the Vice President of Programs and the medical care team to insure that activities related to patient care, supplies, documentation, and the support for staff are optimized for efficient and compliant delivery of high quality patient care and family planning services. The FP Coordinator will round with each clinic site at least once per month to ensure consistent practices and opportunities for training and will participate in outreach activities throughout the year. Responsibilities Responsible for implementing family planning program activities as outlined by Title X Family Planning Program requirements and CBHA guidelines. Coordinate reportable STI follow up with medical providers, patients, and health department. Responsible for purchasing and ordering program supplies and education materials to support the needs of patients and program staff. Provide technical assistance and training to members of the medical care team regarding family planning program and services. Oversee and complete documentation of required information per Title X guidelines. Responsible for the completion of all project reporting, including timely, quality, and concise reports necessary for project deliverables and to evaluate project outcomes. Maintain excellent patient service relationships and represent CBHA professionally in all work related interactions with patients, the medical care team, community partners, and Title X colleagues. Responsible for coordinating and participating in outreach events to promote and educate on family planning services and resources. Promotes a positive work culture and upholds clinic policies and procedures, including the WE CARE Standards of Behavior. Must uphold confidentiality at all times in regards to CBHA activities, reports, financials, patient health information and other proprietary information specific to CBHA. Other duties as assigned.
