A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Implement social work interventions and provide ongoing support aligned with patient needs, resources, and payer requirements. Coordinate care across the continuum, including post-acute placements, hospice, home health, mental health, and community services. Educate patients and families on community resources, healthcare regulations, advance directives, end-of-life planning, and social services. Advocate for patients and collaborate with interdisciplinary teams and community agencies to ensure safe, timely, and effective care transitions. Manage length of stay, level of care, readmission prevention, and care coordination efforts to improve patient outcomes and achieve organizational goals. Maintain professional licensure, ethical standards, documentation compliance, continuing education, and support staff/student development.
Schedule Details Part-time Schedule:
Days. Position is 5 shifts every 2 weeks. Monday and Tuesday each week with an additional day every other week. Occasional weekend and holiday rotation. Occasional cross coverage to Burlington location.
What We Are Looking For License/Registration/Certification:
IL-LSW Eligible, LCSW preferred issued by the State of Illinois/WI-Social Work Certification issued by the
State of Wisconsin Level of Education:
Master's degree in social work from a Council on Social Work Education (CSWE) accredited school.
