Job Description

Job Overview A rehab worker supports residents' well-being and helps develop their physical, social, emotional, and daily living skills. The goal is to strive for independence in activities of daily living while building or bridging interdependence in community social relationships. Services include crisis intervention, behavior management, and life skills training, whether one-on-one or in a group environment. A rehab worker works in a team setting, including case managers, other resident-selected members, and peers. Respect is paid to residents' desires, needs, and preferences. Rehab workers model skill sets, boundaries, offering choices, and problem-solving by looking at options. This position strives to create a fun, enjoyable atmosphere that encourages resident participation and goal achievement. Has sensitivity towards cultural, ethnic, and disability issues. Duties Create daily routines through use of calendars, tracking tools, schedules, alarms, etc. Teach and/or provide transportation to community destinations Make residents aware of educational, physical, social, and/or recreational opportunities available Assist residents with independent living and social skills Keep their surroundings safe and sanitary Ensure that medications are being taken accurately Monitor residents' well-being and report to team members when changes are noted Recognize & respond to potential crisis and emergency situations Write, implement, monitor care plans, and document results Participate in team meetings, report observations, review progress, and make suggestions Ask for help when unable to manage a situation or general concern Follow agency policy Incorporate new learning into job performance
Education/Experience:
Experience working with individuals with mental illnesses
Knowledge, Skills, and Abilities:
Knows Chapter 83 and other applicable WI Statutes Self-motivated and able to function without daily supervision Personal resilience to manage own health and well-being Rapport-building skills of patience, empathy, sense of humor, and a flexible approach Ability to provide information to residents in a direct, but respectful manner Ability to break down activities into achievable goals Ability to observe residents' performance and report clearly to relevant professions Ability to set boundaries and enforce guidelines and rules Ability to respond to crisis situations Team working skills in order to be able to effectively communicate/interact with other professionals, peers, and community members Ability to research available appropriate resident activities in the local area Must be able to pass criminal background check. Pay:
$14.00 - $16.00 per hour Expected hours: 15.0 - 30.0 per week
