Healing House Community Health Worker Madison-area Urban Ministry Madison, WI Job Details Part-time From $24 an hour 21 hours ago Benefits Paid time off Qualifications Bilingual Teamwork Program development Experience working with individuals experiencing homelessness Working with individuals from diverse cultural backgrounds Experience working with individuals from underserved communities Medical terminology Teaching Full Job Description Job Summary The Healing House Community Health Worker plays a vital role in promoting health equity and improving Healing House guest well-being through direct engagement, education, and support. The Community Health Worker empowers individuals and families to access necessary services, understand chronic health conditions, improve health outcomes and navigate complex health systems. Responsibilities Support Healing House guests pre and post discharge to improve access to healthcare, provide supportive services and linkage and referral to community resources. Provide health education on topics such as nutrition, benefit eligibility, health insurance options, including Medicaid and Medicare, and basic life support to promote healthier behaviors Assist clients in understanding public health laws and policies that impact their access to care and services Support program development efforts by collecting data, documenting client interactions, and maintaining accurate records Collaborate with healthcare providers, social workers, and community organizations to coordinate services and resources Participate in community engagement activities to foster trust, build relationships, and promote ongoing participation in health programs Experience Prior experience working with individuals with medical needs, and families who are unhoused. Background in nursing, social work, teaching, or public health program planning is preferred Familiarity with Microsoft Office applications for documentation and reporting purposes Knowledge of public health law policies, Medicare processes, medical terminology, and nutrition principles is advantageous Demonstrated ability in community outreach, client communication, data collection, and community engagement efforts Bilingual skills are a plus to effectively serve diverse populations Experience working with culturally diverse communities and understanding of public health issues related to underserved populations is essential Join us as a Healing House Community Health Worker to be at the forefront of community wellness initiatives!
