(CCR) has been providing treatment foster care services for youth throughout Wisconsin for more than 35 years. We are seeking an experienced Social Worker to join as a Case Manager on our Treatment Foster Care team. This position focuses on supporting youth and foster families through case management, advocacy, service coordination, and relationship-based support. Staff work closely with youth, foster parents, county agencies, schools, and other providers to help youth achieve stability and positive outcomes. At CCR, staff benefit from experienced supervision, flexibility, autonomy, and a supportive team environment that values both the families we serve and the professionals who serve them. This position is primarily a case management and care coordination role. Staff work closely with youth, foster parents, county agencies, schools, and service providers to support treatment foster care placements and positive outcomes for youth. Specific areas of coverage are needed for homes in Dane County, Dodge County, and Jefferson County.
Case Manager Minimum Position Responsibilities:
- Provide case management, support, advocacy and service coordination for youth placed in CCR foster homes.
- Face-to-face contact with foster parents a minimum of once per week
- Face-to-face contact with foster children a minimum of once per week
- Coordinate and facilitate team meetings involving youth, foster parents, county workers, schools, and service providers
- Complete all required documentation, assessments and reports as directed by CCR and required by DCF
- Complete annual foster parent evaluations and provide educational support as needed for assigned foster families
- Attend monthly staff meeting at main office in Middleton
- Provide after-hours On-Call coverage as assigned
- Collaborate/Team with partner agencies
Education/Experience Requirements:
- Master's Degree in Social Work or Human Services related field (In-process applicants will also be considered)
License Required:
APSW or LCSW preferred. Candidates with other relevant Wisconsin human services credentials and experience will also be considered.
- Ideal candidates have experience in foster care, child welfare, juvenile services, family services, case management, care coordination, or related human services fields
