A Family, School, or General Case Manager coordinates the delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs. Provide limited counseling to client and their family.

Job Description

Ensure children in out-of-home care achieve stable, long-term living arrangements by providing support to resource families (foster, respite, kinship or adoptive), birth families and children; provide permanency services to resource families and children moving toward, or having achieved, permanency; balance legal requirements with emotional needs of children and families to facilitate reunification, guardianship, or adoption; achieve goal to provide children with permanent, safe and supportive homes.
Qualifications:
Master's degree from accredited school in social work, Human Services, Psychology or related field; working knowledge of trauma reactions and associated interventions; insurance acceptable driver's license; qualify under WI Caregivers Law, Wisconsin Act 27; successful completion of Juvenile Intake Training; prefer experience in child welfare or social services field. 40 Hours/Week; participate in After-Hours On-Call Rotation. $30.73 - $37.26/hour. Health, Dental and Life Insurance; Paid Time Off; WRS, etc.
