A Family, School, or General Case Manager coordinates the delivery of a range of social and health services to individuals and families. Job Description
Ensure children in out-of-home care achieve stable, long-term living arrangements by providing support to resource families (foster, respite, kinship or adoptive), birth families and children; provide permanency services to resource families and children moving toward, or having achieved, permanency; balance legal requirements with emotional needs of children and families to facilitate reunification, guardianship, or adoption; achieve goal to provide children with permanent, safe and supportive homes.
