Careview Health and Rehab of Minocqua is seeking a caring and motivated Social Worker to join our skilled nursing team and provide support to residents and their families throughout their healthcare journey. The ideal Social Worker candidate will assist with care planning, discharge coordination, psychosocial assessments, and connecting residents with appropriate resources and services. This individual should be highly organized, an excellent communicator, and passionate about advocating for the emotional and social well-being of others. Responsibilities Perform psychosocial assessments for residents and their families Develop, implement, and monitor individualized social service plans Facilitate discharge and transition planning Coordinate with interdisciplinary team (nursing, therapy, medical, dietary, etc.) Provide counseling, crisis intervention, advocacy, and resource linkage Assist with admission evaluations (including PASRR, pre-admission screenings, etc.) Maintain regulatory compliance with state and federal regulations regarding social services in long-term care Lead or participate in care plan meetings and family conferences Provide education, support, and referrals for residents and families (bereavement, community services, financial, legal, etc.) Documentation of social work interventions in the resident's clinical recordRequirements:
Bachelor's degree in social work (BSW) or related human services field; master's in social work (MSW) preferred Current state social work license or eligibility preferred Minimum 1-2 years ' experience in a healthcare, long-term care, or skilled nursing setting Knowledge of Medicare, Medicaid, and long-term care regulations Strong assessment, counseling, and communication skills Ability to work as part of a multidisciplinary team Organizational skills and ability to manage multiple cases Familiarity with community resources and strong referral network Join a team committed to putting people first—every resident, every day.COMPANY BENEFITS
