Position:

Social Worker We are seeking an empathetic and skilled Social Worker to serve as an advocate for our residents, helping them enhance their capacities and coping skills to improve daily life. The ideal candidate will collaborate with staff to assess resident needs, develop individualized care plans, and ensure a supportive, resident-centered environment.

Responsibilities:

Assist new residents and families with admission paperwork and provide guidance on Medicare, Medicaid, and other financial assistance programs. Complete PASRR assessments in compliance with state requirements. Participate in resident assessments, providing social history and background information per the MDS process. Collaborate with residents and families to establish goals and develop written care plans. Lead and participate in Behavior Meetings; monitor Gradual Dose Reductions (GDR) and psychotropic medication consents according to state regulations. Maintain timely progress notes and uphold an open-door policy for residents, families, and staff. Address concerns or grievances, reporting to the Executive Director and initiating investigations as needed. Advocate for resident rights and assist residents in effectively communicating their needs. Participate in Monthly QAPI Committee meetings and daily Stand-Up meetings to report updates and concerns. Lead the discharge planning process, coordinating with external resources for safe transitions. Issue denial letters in accordance with state regulations. Assist in employee orientation and in-service education on resident rights and social service needs. Perform additional duties as assigned by the Administrator.

Requirements:

Bachelor's degree in Social Work, Sociology, Psychology, or related field preferred. Minimum of 1 year of supervised social work experience in a healthcare setting preferred. 1-2 years of experience in long-term care or a skilled nursing facility required. Ability to foster collaboration between social services and other departments. Professional demeanor in-person and over the phone. Strong communication, organizational, and problem-solving skills. Ability to write clear, concise instructions and remain calm in emergencies.
