Job Description

Position Summary The Family Service Coordinator provides direct support and services to children, parents, caregivers, and families participating in Family Support Center programs. This position focuses on building trusting relationships with families, assessing strengths and needs, connecting families to community resources, facilitating goal-setting and family support activities, and promoting protective factors that strengthen family well-being. The Family Service Coordinator serves as a primary point of contact for participating families and plays a key role in family engagement, resource coordination, and service delivery. This position maintains confidential family records and documentation while ensuring services are delivered in a welcoming, family-centered, and strengths-based manner. Key Responsibilities Family Engagement & Direct Services Build positive, supportive relationships with children, parents, caregivers, and families. Conduct family intakes, screenings, and assessments in accordance with program requirements. Assist families in identifying strengths, goals, barriers, and needed supports. Provide ongoing case coordination and follow-up services to participating families. Support families in accessing community resources, including housing, employment, childcare, transportation, healthcare, education, and basic needs assistance. Conduct regular check-ins with families to monitor progress toward goals and identify emerging needs. Assist families with applications, referrals, forms, and documentation required to access services. Promote family engagement and participation in Family Support Center activities and services. Mobility Mentoring & Goal Planning Facilitate Mobility Mentoring and other family goal-setting processes. Assist families in developing individualized goals and action plans. Support families in identifying steps toward increased self-sufficiency and stability. Maintain accurate records of family goals, progress, referrals, and outcomes. Monitor participation and provide follow-up encouragement and support. Program Facilitation & Family Activities Plan, coordinate, and facilitate parenting workshops, support groups, playgroups, family engagement events, and educational activities. Create a welcoming and inclusive environment that encourages participation and relationship-building. Collaborate with community partners to provide educational opportunities and resources for families. Support parent leadership opportunities and encourage family voice in program planning. Resource Coordination & Concrete Supports Assist with pantry operations as needed, including helping families access food, hygiene, baby, and household supplies. Maintain knowledge of community resources and referral networks. Assist families in obtaining services and supports that promote family stability and well-being. Documentation & Confidential Records Maintain accurate, timely, and confidential family records in compliance with agency policies, grant requirements, and applicable privacy regulations. Complete required documentation, case notes, service records, referrals, and outcome tracking. Ensure secure storage and handling of confidential family information. Assist with data collection and reporting requirements related to family services and program outcomes. Other Duties Participate in staff meetings, trainings, and professional development opportunities. Follow all agency policies, safety procedures, and confidentiality requirements. Perform other duties as assigned to support Family Support Center operations and family success. Competency Qualifications Education Minimum requirement is a High School Diploma or equivalent. Experience Experience working with children, families, community services, case management, family support programs, or related human service fields is preferred. Language Ability Must have the ability to communicate effectively in English and establish positive relationships with families, community partners, and staff. Reasoning Ability Must have the ability to exercise sound judgment, maintain appropriate professional boundaries, and apply problem-solving skills when assisting families. Computer Skills Must have working knowledge of Microsoft Word, Excel, Outlook, electronic databases, and recordkeeping systems. $32,000.00 per year
