Accept and provide care coordinator to children and families with a caseload of 8-10. Intensively engage and meet with youth, their families, and the wraparound team. Facilitate family team meetings (including meeting arrangements and documentation) at least monthly. Complete face-to-face family meetings with the child and his/her supports at least weekly. Complete an initial Child and Adolescent Needs and Strengths assessment within 14 days. Make all attempts to facilitate initial contact with a family within 48 hours of referral. Maintain professional, appropriate, collaborative relationships with community providers and local WVDHHR, probation, circuit courts, etc. Provide training on wraparound process to family team members to facilitate appropriate participation and care. Requirements:
21 years of age, valid WV driver's license and insurance and registration, pass pre-employment drug screen, pass criminal background and CPs check, have a degree in a human service field. Will work to support families and children following SAH guidelines with the DHHR, complete assessments and reports, face to face family meetings weekly, maintain professional and appropriate relationships with community and local DHHR, probation, and court system.
