JOB SUMMARY
The Case Manager role is defined as a collaborative process used to assess, plan, implement, coordinate, monitor, and evaluate options and services to meet individuals' health needs through communication and available resources to promote quality, cost-effective outcomes.JOB QUALIFICATIONS
Education:
Registered Nurse with an AD, BSN, master's degree, or doctoral degree Social Worker with a bachelor's degree in social work, master's degree, or Doctoral degree in Social workCertifications/Licenses:
Registered Nurse with a license to practice in Wyoming. LCSW with a license to practice inWyoming Job Knowledge and Skills:
Five years' clinical experience Two years' experience in case management/utilization management preferred. InterQual application preferredBenefits
